More angioedema from instant coffee, but not from caffeine

Beverages are infrequently implicated in causing angioedema and urticaria in adults. There are few reports of the association of coffee with angioedema and urticaria. Accordingly, the case description by Tognetti et al. is a valuable addition to the medical literature. A similar case was reported by us in 2012. In that earlier instance, a 50-year-old man presented with angioedema of unknown cause. By self-challenge, he identified a caffeinated instant coffee as the trigger. However, the caffeine was almost certainly not the culprit. The results of the self-challenge precluded any pressing need for immunoglobulin or other analytical testing. The patient eliminated that coffee product from his diet and had no recurrence of angioedema.
The detailed description of the serologic and allergologic tests that Tognetti et al. performed for their patient would have been of use in the earlier case, had such testing been advised. Their description serves as an excellent reference for clinicians who seek to identify triggers for angioedema and urticaria. The authors and the Journal deserve commendation for publishing this valuable article.
